First time hockey players who have never played hockey before
will be charged a NON-REFUNDABLE $50 REGISTRATION FEE.
However, all other volunteer requirements are mandatory
In order to complete registration ALL of the following are required:
1. ONLINE REGISTRATION
2. MEDICAL FORM - can be found on website under MMHA Forms
3. REGISTRATION FEE paid in full, installments, or payment plan
discussed with the registrar
4. DEPOSIT CHEQUES - not dated.
These are only a deposit for your required fundraising obligations
Once requirements are completed the cheques will be returned
- 1 x $100 TOURNAMENT deposit cheque per player
- 2 x $100 VOLUNTEER deposit cheques per player
- $200 BINGO deposit cheques per below:
1 child - 4 cheques
2 children - 7 cheques
3+ children - 10 cheques
5. RESPECT IN SPORT - Each family must complete the Parent
- a one hour course which costs $12 + GST

- must be completed prior to Any player being allowed on the ice.
6. Bingos are in good standing from last season - they must be worked
or payment made
Players will not be allowed on the ice until
registration is completed in FULL
Medical Forms must be printed off, filled out, and handed in separately
Bingo, Tournament & Volunteer deposit cheques must follow registration
How to complete registration:
1. Contact the registrar to make payment arrangements
2. Mail your medical form, deposit cheques, and payment to:

Box 146, Millet, AB, T0C 1Z0
3. Attend a registration evening to hand all paperwork and cheques in.
If you registered your child online last year: Select "already have an account" and login using your email and password. There is a link to click if you don't remember your password that you used when you set up the account - look for "Forgot your Password" - Do not create a new account, please contact the registrar if you can't retrieve your old password.
If you did not register your child online last year: (Note that the registration site is different than the family login on website). You will select "New to the Site?" and begin the process of setting up an account. Enter all required fields on the page with your personal contact information. The personal information that you enter in this area is your own, not the registrants (your child). Check the "I am also a Guardian/Emergency Contact" checkbox. If you are the registrant's legal guardian, this will ensure that you will not have to re-enter the information later on in the process.
Once you have submitted your information, you must check your email to continue with the registration process. Please make sure to check your junk/spam email folder, as it is common for the activation email to get sent to this folder. In your activation email, click on the link within to activate your account.
Once your account is activated you can log in and register the player. Login using your email and password. The first time you register, you will have to enter in the child's information, and the system will store this information for next year.
Please note that we accept credit card payment online. You must print off the medical form, fill it out and submit it seperately.
